In patients with systemic lupus erythematosus and connective tissue diseases there is an increased risk of aseptic meningitis.
The effect on the digestive tract. NSAIDs should be used with caution in patients with chronic inflammatory bowel disease (ulcerative colitis, Crohn's disease) as these conditions may be exacerbated. There have been reports of cases of gastrointestinal bleeding, perforation, ulcers, arise at any stage of the treatment of NSAID regardless of warning symptoms or presence of severe violations of the digestive tract in history.
The risk of gastrointestinal bleeding, perforation, ulcers increases with increasing doses of NSAIDs in patients with ulcer history, especially complicated by bleeding or perforation, and in the elderly.

These patients should start treatment with minimal doses. Caution must be exercised with care in patients receiving concomitant medications which could increase the risk of gastrotoxicity or bleeding, such as oral corticosteroids, anticoagulants (such as warfarin), antiplatelet agents (for example, acetylsalicylic acid). With prolonged treatment for these patients, and also for those in need of concomitant use of aspirin in low doses or other drugs that may increase the risk for GI, a physician should be considered the appointment of combination therapy with misoprostol or proton pump inhibitors.
Patients with existing gastrointestinal disorders history, especially the elderly, should report any unusual symptoms from the gastrointestinal tract, especially gastrointestinal bleeding, at the beginning of treatment. In the case of gastrointestinal bleeding or ulcers in patients receiving ibuprofen, the treatment should be stopped immediately.

The effect on the kidneys. Long-term use of NSAIDs may cause a dose-dependent reduction in prostaglandin synthesis and provoke the development of renal failure. High risk of such a reaction are those with impaired renal function, cardiac impairment, liver dysfunction, those taking diuretics and the elderly patients. Such borlnyh want to monitor renal function.
